20. Care Management

Assisting a client to identify and utilize services needed to assure that the client is receiving, when reasonably possible, the level of care that best matches his or her level of need. The Care Management Unit, through its Care Management Unit Supervisor and staff of care managers, assists clients with services as specified in the [Care Management] Act, including ongoing consultation, assessment, Long-Term Care Plan development, and referral for clients in need of long-term care; coordination of the Long-Term Care Plan; monitoring of the delivery of services for clients, and review of the client's Long-Term Care Plan.

Additional considerations for time study:
This code should be used for all activities related to care management, including:
  • Care management oversight and coordination of direct care services provided under State Aging Services, such as senior center hours, social activities, and respite
  • Paperwork
  • Clerical activities, such as making copies
  • Time spent reviewing voicemails or emails
  • Time spent consulting with a supervisor or supervising 
  • Travel including time spent arranging transportation
  • Time spent entering information about the activity into a database or software program
  • Calls, emails, or other communications to obtain clarification of an inquiry

Time study codes:
  • 20a. Enrolled in Medicaid- Assisting an individual who is currently enrolled or applying for Medicaid.
  • 20b. At risk of spenddown/institutionalization- Assistance may prevent or delay the individual from entering a nursing facility or other Medicaid-funded institution. It must be documented that the individual must meet the Spenddown Worksheet criteria.
  • 20c. Non-Medicaid- Assisting an individual who is not currently enrolled in or applying to Medicaid AND is not at risk for spenddown and/or institutionalization.
